Charlie is a 49 year old Black female. Charlie lives in Atlanta where she is a licensed professional counselor (LPC) and active member of the community where she spends at least one day a week volunteering at the Atlanta Humane Society. She and her partner Evelyn have been married for ten years and have one adopted and one biological child. Evelyn is a stay-at-home mother for the couple’s two children. Charlie has a thriving private practice and makes additional income coaching woman’s lacrosse. Charlie describes her life, marriage, and career as “happy” and “successful”. Charlie states the areas she struggles in are diet and smoking. She states “but I know this already, I’m a therapist too!” Charlie tells you in your first, brief session at a local outpatient medical clinic, that she “wants to quit smoking and eat better but can’t”, because “I’m too busy and smoking keeps me calm, especially at night”. Charlie then states that she has been a smoker most of her adult life, she quit when the children where first born/adopted, but picked back up a year or so later. When it comes to food, Charlie says she eats “whatever is convenient or quick.. mostly burgers”. You speak to the attending physician who says that Charlie’s diet, smoking, and family medical history put her at great risk for a heart attack or stroke. The doctor continues by saying “her blood pressure is out of control, she is playing with her life at this point.. see what you can do..”. You have this visit and Charlie’s next couple of doctor visits to engage. Visits are about 60-90 days apart. Your job is to ready Charlie for behavioral change then elicit said change.
