Grant Proposal Exercise Assignment
Overview
You will continue
to work in your groups in order to write a grant proposal. Based on the
template provided in Chapter 14, you will request funding for the practice initiative
developed over three weeks. Be sure to be persuasive and use citations from the
literature to back up why your initiative will work.
Include at least 4
peer-reviewed referenced in addition to the course textbook and the Bible.
The grant proposal
must include the following: (see Chapter 14 of the textbook for the components
of a proposal)
· Executive summary (1
page)
· Statement of need (1
– 2 pages): include target population, purpose, goals, and significance of
initiative (what defines success)
· Brief overview of organization
(1 page): this refers to the original organization that the practice initiative
was linked to during Module 3: Week 3 (e.g. Cleveland Clinic, etc.).
Project description
(2 – 4 pages): problem statement, describe project, statement of the problem,
how the project will meet the need(s), procedures, resources,
deliverables/projected outcomes, evaluation plan, and dissemination plan
