Part I: Population Health
a. Identify a specific population and community that will serve as the focus of this paper.
The population should be at the state, county, or city level.
b. Provide background information (statistics, sociodemographic, age, education, identity) of the selected population.
c. Discuss the specifics of the target population.
§ Identify their behaviors, health risk factors, and vulnerabilities.
d. Describe the overall health status of the community, the population’s health risk factors and their top needs (a minimum of 3 factors must be identified).
§ Provide statistics, incidence, prevalence of the health issue
§ Discussing the trend (moving upwards, downwards, or plateauing)
e. Explain and highlight one of the health risk factors identified and focus on one specific need of this community.

Part II: Identify a Community Organization/Facility
a. Discuss the current assumptions/perception of the public health issue
b. Select a local health care facility or a community-based organization that is addressing the need within a local community identified in part I.
§ What is the purpose of this organization?
§ Identify how they are addressing the need and the health problem.
c. Discuss what actions are being taken and at what socioecological level (individual,
interpersonal, community, policy) these actions are being implemented on.
d. Explain and discuss if these actions are having an impact. Provide support for your reasoning.

Part III: Critical Thinking
Respond to the following questions:
a. How can community-based organizations address population health issues?
b. How would you find out about the health issues of your community? Identify a variety of sources.
c. Discuss recommendations for change (policy changes? health behavior changes?)
d. What role do health promotion wellness professionals have in addressing population
health issues?
