For our main activity this week, you are going to compose your own scenario/case study. As health professionals, it is important to know how to summarize patient and family situations. The social history provides an important foundation for the entire team – the information we gather during our initial intake interview or patient assessment is so valuable. It takes practice because we want to include enough detail to be helpful and yet we know we can’t write lengthy reports in the chart.
As such, each student is going to write their own fictional scenario. I will review these and provide feedback to you. You will make any necessary revisions and resubmit to me so that we will use these scenarios as the basis for our next Let’s Chat: What Would You Do discussion in two weeks. At that time, you’ll each choose someone else’s scenario to use as the basis for locating appropriate referrals and you’ll have a chance to offer feedback about the scenario as well. This is meant to be fun and a great way to practice summarizing key information in the patient record. Please use the following criteria as you develop your scenario:
1. Your scenario should be just 1-2 paragraphs in length.
2. Include pertinent demographic information: race/ethnicity, age, gender, family structure, health needs (physical, emotional, social, spiritual, etc.), family dynamics that impact functioning, social determinants of health, etc.
3. Have fun and be creative! We want these to be unique, but also not so hard that people can’t find any resources to help. Try to be realistic and provide enough detail for them to get a good start.
