Exploring a knowledge-based map: Open a web browser and access the I-Magic Mapping tool at http://imagic.nlm.nih.gov/imagic/code/map. Review the map records in table 17.7 of your textbook. Read the instructions for use of I-MAGIC on the website. Enter the following problem in I-MAGIC: Contusion of chest (10050004). Then choose “Update list.” Obtain the suggested ICD-10-CM codes for the encounter by selecting “Get ICD Codes.” What element of context does I-MAGIC require you to enter before it responds with a final recommendation? Did you enter any context for the gender or birth date of the patient? Did it make any difference in I-10 coding in this case? How many I-10 codes were recommended?
Next, add a second problem to the list: Sternal fracture with retrosternal contusion (448321000124104) and again select “Update list.” This adds a comorbidity as an element of context. Get the I-10 code list once more. How many I-10 codes are recommended for each principal diagnosis? From the rule list for this mapping of chest contusion, can you see why adding the comorbidity resulted in a secondary code whereas there was only one code listed at first?
Clear the problem list and input the following problem: Anemia of gonadal dysfunction (22347002). Explore how the rule-based map handles context of gender. Note the differences in ICD-10-CM codes assigned for males and females.
Clear the problem list once more and input the following problem: Bacterial sepsis (10001005). Explore the differences in I-10 coding when you enter a patient birth date within the last 29 days and otherwise to assess the management of age context within the map. Are the suggested codes different for newborns as opposed to adult patients?
Explore your favorite I-10 coding conundrum and prepare a report with your observations from these exercises. Discuss the map and its utility. What is the source? The target? Map type? Cardinality? Techniques for context management? How does managing context affect the specificity of the I-10 coding? Are there advantages to more specific diagnoses?
