Each team is requested to consider
one of the following types of safety problems:
1- A major safety or
environmental hazards that could happen via multiple scenarios, or
2- A system that has several
components with potential modes of failures.
*Use one of the following
two methods, as appropriate, to analyze the hazard(s) root causes: (1) Fault
Tree Analysis (for the first type of problems), or (2) Failure Modes and
Effects Analysis (for the second type of problems).
*Use the results of the
analysis to prioritize control measures.
