CC (chief complaint): “I’m seeing weird things”
HPI: a
45-year-old Latina woman who has come to the clinic due to she has seen weird things she mentioned that
she has seen weird things, such as
seeing spiders on the walls. Additionally, she is afraid that the spiders
will bite her. In response, the patient avoids leaving the house, using the
phone, and watching TV programs. As a result, she was unable to maintain a
job during this time and reports that she is arguing with her husband. The
patient reported that he experienced those symptoms for three months. She was
diagnosed with anxiety at the age of 30 and prescribed buspirone 10 mg twice
a day. she did not like taking any oral medications, so her care provider
respected her opinion and conducted psychotherapy sessions with her. she
reported she drink 6 beers daily and she smokes 10 cigarettes daily. she
reported feeling sad after losing her job and disturbance in her relationship
with her husband. she also reported difficulty sleeping, feeling weak,
fatigued, and nightmares she denied and thoughts of committing suicidewas
scheduled for next week. A patient will be continuing outpatient
psychotherapy.
