What were the measures that were established prior to initiating the project?

Please use the following

Form the
Team: Who in the organization provided the impetus
for the project or the leadership as it moved forward? (Not one person’s identity
but rather what discipline and what level in the organization). Who was on the “team”?

Set an
Aim: What was the aim of the project – was it time
specific and measurable?

Establish
Measures: What were the measures that were established prior to
initiating the project? (i.e., the
feedback to know if the change actually led to an improvement)

Identify
Changes that were made: Describe
the actual change(s) that were being tested or tried out. Were change ideas supported by literature?

Test
the Changes: Describe
the PSDA cycle. (plan, do, study, act) If PDSA cycles are not evident, please acknowledge.

Implement
Changes: Describe how changes were implemented. Once
the changes were tried on a small scale, were the changes implemented on a
larger scale?
Spread
the Changes: Was there anything further done in order to
expand the protocol beyond the original setting of the project?

Project
Critique: Note any policy changes
that led the organization to focus on this project. If there were no policies
driving this project, what potential policies could you identify that would
facilitate the success or failure of the project? What was done well, not done
well? What would you have done differently? Was the project successful or not?
Why? Have changes or improvements been sustained over time?
