Case study: In their discussion of Health Home, one of the three reform models for patient care, explains that their model is built upon preexisting conditions of medical home care based on their state experience. My question is why isn’t it universal, when certain states have more uninsured patients with limited accessibility to standard healthcare, such Florida or Georgia, does that mean not as many recieve treatment in those states as well?

Reading Assessment: In chapter 3, pg. 82 it states that the implication of PHR systems can have a positive impact on patient quality, but certain demographic groups haven’t seen that same positive trend analysis . Describe some key characteristics of a “Plan Centric EHR”?
1. A Library of Plans That Cover a Wide Range of Situations – examples for plans are managing hypertension, removing an appendix, losing weight, and treating cervical cancer. Though all there plans may have variations based on patients needs
2. Algorithms to Form a Patient’s Master Plan – a master plan will combines the variations of what the patient may already have and find the best solution to treat what is need. The system will then select the optimal plan for success based on patients needs
3. Team-Based – These covers the steps need to be carried out for patients with all the people involved within the patients care to where the members of this plan can communicate and set tasks for one another
