The goal of this assignment is for you to increase your awareness of the variety human services providers to elders, their contact information, and some basic information regarding the functioning of various agencies.
To begin, identify a type of service elders need that is of interest to you.
Here are some examples of types of services:
legal services; income assistance/management; medical services particular to specific health conditions; hospice/grieving; mental health; housing/continuum of care; homelessness assistance; housekeeping/nutrition/assistance with ADL; therapeutic services; recreational services; fraud/exploitation victimization (non-relative perpetrator); neglect/exploitation/abuse victimization (perp is relative or care provider); care provider support/assistance; public information, policy making, advocacy.
This list is not exhaustive and is meant to provide suggestions for your choice. You may choose to focus on an area of service not listed.
Chose an area of elder service provision, do research to identify approximately 5 local agencies/programs which provide these services.
If there are not 5 local service providers, you may write about providers in other communities, or those that serve state or nationwide. Your research should be done by questioning employees (and possibly clients or volunteers) of the agency/program and reading print or electronic material.
For the written portion of the assignment, you must write the following information for each of the agencies/programs (each of these pieces of information must be communicated to the client, in order to provide an informed, competent referral for any client):
The official or legal name of the agency/program;
The physical address, telephone number, web address, contact person, any requirements for clients to receive services (such as fees, documents needed, pre/corequisite services, professional referrals, etc.), hours of service availability;
Describe the client population served;
Describe the services provided;
Describe any barriers to service provision or barriers that prevent the agency from accomplishing its goals.
List other service providers each agency works in tandem with to service clients.
Take care to write your assignment using proper grammar, sentence structure, and punctuation. Social workers MUST have excellent written and oral communication skills because our communication can impact peoples’ lives.
