IEP Transition Plan

This week, using the list of assessments, your research and Amanda’s IEP present level of performance and transition pages, create a revised IEP transition page OR a narrative transition plan for Amanda. Consider her a high school student at this time.You must look up each assessment to determine if it is suitable for Amanda, considering her present level of performance and post-secondary goals. You may use as many assessments as you like, as long as you have a sound transition plan for Amanda. Please address each assessment that you select and share the information from the IEP that supports why you selected the assessments.
Do not address the assessments that you do not select. You may present your information as a transition page in the IEP or as a narrative, however the transition page is much preferred. If you choose to use the IEP transition page format, please include a cover sheet and a reference list. If you choose a narrative form, please use the APA format for your paper, including all of the items that were required for the IEP Strategies paper. If you are using Amanda’s transition pages in the sample IEP as a guide, please improve on that based on your research, and do not submit an identical transition page.

I have cut the IEP transition pages that I want you to complete and added notes to each section that needs to be updated. Before developing the goals, you will need to select 3-6 appropriate transition assessments and update that section at the very top (use the resources provided in the folder unless you already know some that would be appropriate for Amanda). A good transition plan will have goals in the areas of education, employment, training and independent living. Your plan for Amanda must have goals and activities in each of those sections.
The key is selecting assessments and goals that are appropriate for her specifically. Then review the areas of need below the goal. You must have something under instruction for all of them, but also consider the other areas to determine if activities should be added. You can add fabricated due dates and individuals assigned to each section.
