Please submit the health information technology (HIT) idea/topic your team will be researching and presenting to address the triple or quadruple aim. Each team member should view the Team Project Overview lecture prior to working on this assignment. Please do not use quotes in this assignment. Each Team should submit 4 pages as described below, 12 point font, double space, and proper grammar/spelling:
Page 1: The title page with team number and all team member names.
Pages 2-3: A 2 page description using the following headings below:
Clinical or public health problem: Description of the clinical or public health problem that the health information technology (HIT) your team selected will address (3-5 sentences). Must mention which aspects of the Triple/Quadruple Aim the HIT project will address.
HIT approach and setting of project: Description of the basic idea of the HIT approach your team will take to implement the innovation to address a health problem (4-5 sentences).
Consider in your write-up the target population, the setting (e.g. home, clinic, hospital, nursing home, etc.), technology requirements (type of software and hardware), at least 3 possible outcome measurements of project success-refer to smart measure slides.
Health informatics area: Which are of HI does this technology project fall under and why (e.g. clinical informatics, consumer health informatics, public health informatics, clinical research informatics etc.) (2-3 sentences)
Users: Description of at least 3-4 HIT potential users of this technology (e.g. administrators, technicians, clinicians, patients, consumers, public health officials etc.)
Page 4: 5 or more references (APA format preferred) with full citations not just URLs.
