For these two assignments you will need to create your own original Assessment form. The assessment is different than the Screening tool as the assessment is meant to obtain more information. Specifically, you will need to create an assessment which includes questions about:
Client’s history of drug use throughout his/her lifetime
Client’s family history of drug or other alcohol use
Client’s history of legal problems
Client’s history of medical/health problems
Client’s history of mental health problems
There is no required length of pages but just be sure your assessment has at least one or two questions for each section above. Include at least one close-ended and one open-ended question for each section; each section will be worth 2 pts each. Make sure the assessment is completely ORIGINAL and not just copied from elsewhere.
