• Thinking, Development, Correctness of Style, Use of Sources and Format
Demographics- patient is 75 year old male, admitted to the hospital for generalized weakness and has been diagnozed with gerd. I will add the rest of the info myself.

• Brief pathophysiology of admitting diagnosis
• Admitting Diagnosis
• Surgical Procedure and date if applicable
Assessment of Patient Learning Needs
• Readiness to learn
• Baseline knowledge
• Barriers to learning
• Patient’s level of internet use/ computer accessibility
Nursing Diagnosis and Learning Outcomes
• Identify a NANDA Nursing Diagnosis that you find applicable to the chosen patient. Use the Ackley (2020) Nursing Diagnosis Book.
• For example, if the patient has a medical diagnosis of cholelithiasis. Use the index for medical problems, go to p. 39 in book, then select one of those nursing diagnoses, such as- acute pain. Patient teaching will focus on pain management.
