developing a health education/public health evaluation plan and a corresponding logic model, develop a Program Evaluation Plan. It is important to have a solid evaluation plan to see if the programs actually impact health behavior. Therefore, think about the health education/public health program you want to implement and evaluate, and use the Centers for Disease Control and Prevention’s Framework for Program Evaluation located under the Assignment’s tab to guide your Program Evaluation Plan. Respond to the following questions and topics outlined for each step:
Step 1: Identify your Stakeholders
Step 2: Describe the Program including:
a. A brief statement of need to identify the health issue addressed
b. Inputs or program resources available to implement program activities
c. What theory/model will you use to link program activities with program outcomes
d. Is the program school-based, hospital-based, community-based, etc.? How many participants are involved – is it large or small?
e. What type of program will it be? Is the program traditional face-to-face, online, or a large state or national program, etc.?
Step 3: Focus the Evaluation
a. Create evaluation questions and program objectives. Make sure program objectives are S.M.A.R.T. objectives and related to participants’ knowledge, attitude/beliefs, and behaviors.
b. Discuss budget and resources you will need
Step 4: Plan for Gathering Credible Evidence
a. How will you evaluate the health program to best answer your evaluation questions/objectives? Will you use quantitative or qualitative methods? Or both? (e.g., surveys and interviews)
b. How will you access your population? What would be the method used to recruit a sample?
Step 5: Plan for Conclusions
a. What is your timetable?
Step 6: Plan for Dissemination and Sharing Lessons Learned
a. Who will you share your evaluation results and lessons learned with?
b. How will you communicate and disseminate findings?
