What did you think of the processes for evaluating whether you can release records and responding to records requests?

In the first records assignment, you took a consumer perspective, and attempted to access your
own healthcare records from providers.
In this assignment, you’ll take the provider perspective,
and consider requests for records release.
To release an instance of a medical record, the following conditions must be met:
1. The authorization form must be complete, including all core elements described in the
checklist for valid authorization (linked in Canvas).
2. The medical record under consideration for release must meet all criteria described in
the authorization form.
To gain an understanding of the regulations around records release, visit this site and read
through the information in the menu on the left of the web page (shown below):

You may also find other sources of information on records release in your own research. Please
cite these sources in your report.

For each patient, consider the associated medical record and authorization form, and then
do the following:
1. Make a decision about whether you can release the patient’s record, given the
information in the authorization form and your knowledge of the regulations for release
of records.
2. Write a clear and complete explanation of why you believe that you can or cannot
release the record, based on the authorization form.
3. Write the body of a letter that (a) explains why you cannot release records; or (b) serves
as a cover letter for delivery if you can release the record. For cases where you cannot
release records, be sure that your letter does not reveal the existence of any records
that may exist.
Second: write a report that describes your experiences in these activities. Specifically:
• What did you think of the processes for evaluating whether you can release records and
responding to records requests?
• Compare and contrast your experiences in the first and second assignments. Would you
change anything from your first submission, based on your experience here?
