Provide a brief description of the setting and your role within that setting. What is your primary responsibility to this family?
Identifying Information (5 point)
Identify basic demographic data such as the client’s age, gender, race, ethnicity, education, income, employment status, household members and living arrangements.
Reason for Referral (2 point)
Provide information on who referred the client to you, when the referral was made and what that referral agent expects from you.
Relevant History (5 points)
Provide a short history of the current situation. Include relevant information (including developmental, health, family, and community) and past strategies used to address the situation.
Assessment (18 total)
1. Identify the client issue(s) and needs. Describe the consequences or impact of the issues on the client’s life. (3 points)
2. State a hypothesis to explain the client and the issue(s). Using a theory or approach, provide your rationale for why the issue exists. What individual, family, community, societal and historical factors play a role in creating or continuing the issue(s)? (3 points)
3. Identify the strengths and resources available to the client to bring resolution to the issues(s) you have identified. Include both the client’s internal strengths and external resources available in the environment. (3 points)
4. What additional information do you need to better understand the client and issue(s) or to develop a more detailed intervention plan? (3 points)
5. What do you need to consider before you begin working with the client? Consider your characteristics such as race/ethnicity, gender and the impact they may have on the social worker – client relationship. In addition, what knowledge do you need to develop before working with the client? (3 points)
6. Complete an ecomap, genogram and/or a culturagram that illustrates the strengths and challenges experienced by the family. (3 points)

Points will be earned based on the care with which the paper is prepared specific to appropriate sentence structure, grammar, spelling and punctuation. Proofreading and editing is vital as your credibility as a social worker will be judged based on your ability to present information in writing.
