The main objective of this project is to develop a change plan in your clinical area incorporating Transformational Leadership Theory Concepts as the theoretical framework.
1. Identify a nursing care issue/problem in your clinical area that needs to be changed working along with your Faculty mentor/preceptor who agrees with your project.
2. Determine what type of change you want to produce as a leader:
a. Attitude, Behavior, knowledge, skills, and/or performance?
b. Are you targeting process or structure aspects of the nursing care delivery? c. You need to consider these factors in order to develop your plan and use the
appropriate theoretical framework as guidance.

This project is on pressure injuries.
There has been an increase in pressure injuries that are missed on admission and pressure injuries that have worsen during hospital stay and some patients that are bedbound has developed pressure injuries. Educating staff is important and ensuring they follow the intervention in place are also important

Some intervention in place are turning team every 2 hours, performing skin assessment on admission and taking pictures of any skin break down and stage it. Overlay air mattress, consulting wound care for ulcers, following dressing change orders, keeping skin moisture free, braden scale
