Write a journal by describing what you did and learn in the paragraph as follow:

a) You are doing three weeks internship as a health advocate in a hospital, and your job consists of the following :
Helping patients in the waiting room complete a brief, one-page survey that asks about various Social Determinants of Health; for patients that speak a language the volunteer does not know, using an interpreter phone to communicate with the patient;
· Once patients have completed their visit with the clinical team, referring patients to appropriate resources based on their responses on the survey and a table of resources;
· Make other referrals as necessary using NowPow/PowRx online referral software tool;
· Referring patients to social services at Lincoln or in the community;
· Entering data information from the completed survey and about the referrals made into a spreadsheet on the laptop for analysis; and
· Assisting in other aspects of the workflow, such as follow-up calls to patients who prefer to have the referrals made at a later time.
b) to facilitate the job, your manager sent you to the epic training such as the Epic patient portal application (my chart), Epic’s operating room management system combining clinical & decision support tools with integrated scheduling for the management of hospital operating rooms & ambulatory surgical centers.
