Application: Standardization and the Electronic Health Record
A still-evolving area of informational standardization in health care involves the electronic health record (EHR). Although strides have been made with regard to standardizing the content of EHRs, the structural standardization of these records is still in its early stages and, as such, their actual interoperability remains limited within the overall practical framework of health information exchange.
To prepare for this Application Assignment, using the Walden Library databases as well as the Internet, conduct a literature review on the Continuity of Care Record (CCR) and the Continuity of Care Document (CCD), two commonly used structural standards for patient health records. Then, review the perspectives on electronic health record structural standards that were offered by the presenters in this week’s video.
To complete this Application Assignment, write a 2- to 3-page paper based on your findings, describing the structural components of the CCR, including the specific sections in its core data set, as well as some of the optional data sections that providers can choose to include. Then, detail at least three potential benefits of the CCR. Lastly, explain the how the CCR relates to the CCD.
