Examining HFACS

Go to the NTSB website on Aviation Accident Reports (Links to an external site.) and select one aviation accident report that particularly interests you. It must have a “Factual Information” chapter.

Open the PDF file. Review the “Factual Information” chapter (do not review the Analysis, Conclusions, or Recommendations chapters) of the Full Report (not the Summary).

Then analyze and evaluate the human error aspects of the accident by creatively applying the data analysis tools of the Human Factors Analysis and Classification System (HFACS).

Report your results and conclusions in a two-page paper (not including cover and reference pages). Remember to always support your work with a reliable source(s). APA formatting is required.
