Assignment 7-1: Adding Clinical Subforms to PHR Tabs

1. Copy file and paste as save as new version. Open this version
2. Go to Design View.
3. Open Personal Health Record and design view.
4. We are going to add the Medication and other subforms.
5. Right click the Contacts tab and select Insert Page.
6. Grab Medications subform and drag and drop it into the form.
7. Delete the Medications Subform header as you don’t need it.
8. Align your form within the tab. Leave the yellow line showing.
9. Right click the yellow line/bar border, the click Properties.
10. Click Data and find Link Master Fields and Child Fields.
11. Click the three button command on Link Master Fields.
12. It will pull up a Subform Field Linker. It should have Personal ID in Master Fields and Personal ID (FQ) in Child Fields. Click OK
13. Click on the yellow bar again and go to Properties, click theTab page number and change to Medications.
14. Click on Form View and be sure everything worked.
15. Now you will make the Personal ID Foreign Key not visible.
16. Go to Design View, go to Medications Form, highlight Personal ID (FK), Click on Properties Sheet and highlight visible and select No.
17. Test and make sure the subform works by opening the Personal Health Record, go to Patient 3 and test this by entering a new medication.
18. Go to Medications table and make sure the new medication is there and that it is for the appropriate patient.
19. Repeat this process for Allergies, Conditions, and Immunizations.
Assignment 7-2: Adding a Patient Using Forms

1. Copy file and paste as save as new version. Open this version
2. Go to Design View.
3. You will add one complete patient to your database. You will need this for your final assignment.
4. Open Personal Health Record. If you want more room you can close the navigation bar.
5. Add a new record by clicking the arrow with the asterisk.
6. Add a new record with two allergies, two medications, two conditions, and two immunizations.
7. Test it and make sure it works.
