In your project, you should
address each of the following points:
Describe
the community you selected. Be sure to include information about
why the community should be focused on and what some of the
population’s unique characteristics are (e.g., location, behaviors,
socioeconomic status, culture, etc.). Be sure to rely on scholarly sources
when describing the population rather than your own personal experience or
opinions.
Explain the
determinants of health for the selected population. That is, explain why
the selected community experiences higher rates of particular health
issues and what is influencing its health status. Consider the
social determinants of health for this community based on a health equity
approach.
Describe the health
issues affecting the community using the epidemiological approach
(see Chapter 2 of our book). Be sure to include information about what the
health issues are and the prevalence and incidence of the health issues in
the community. Focus specifically on those health issues that are
more prevalent in the selected community than in the population
overall. Use the information in the second bullet related to social determinants
of health to help explain why these health issues are more common among
this population.
Select one health
issue that is particularly prevalent in the selected community and
discuss this particular health issue in depth. Be sure to include
information about what the health issue is, how the health issue develops,
what happens to a person who has the health issue, and why the community
you selected is particularly affected by it.
Explain this
community’s health in relation to the levels of prevention (the spectrum
of primary prevention to treatment). Discuss how prevention efforts
and access to quality health care (or the lack thereof)
affect this population’s health.
Discuss how the
structure and coordination of the American health care
system contributes to this health condition among the population. Discuss
how the health care system can/should be altered to support value-based
and coordinated care, ensure there is a viable workforce, and engage
patients and the community to address this health issue.
Discuss future trends
for the community’s health and suggest a strategy for improving the
health of the community based on the four pillars of population health.
Consider what would be the best way to improve the health of the community
from a population health standpoint given what you know about it and
what we learned in class. This strategy should be based on current
research about the community and what the scholarly literature says is the
best way to address this health issue considering the four pillars of
population health.
PHP Part 1: Presentation Screencast
For part 1 of the
assignment, you will create a six- to eight-minute presentation via
a screencast of your PHP project. (Please see the separate
section on Creating Screencast Presentations.)
Your screencast should address each of the bullets in the project
overview above (recommend one slide per bullet).
After
the screencast has been created, you should have a link to
the screencast from the screencast website or it can be
uploaded to a video sharing website, like YouTube or Vimeo. This
is the link you should submit for your assignment, along with the PDF of
your presentation slides and your discussion question. You can
put the link and discussion question in the comments box of the assignment
page in Blackboard. If you do not submit these three things as directed, I
will not accept the assignment and will ask you to resubmit it.
Submission guidelines:
For this assignment
you should submit three things in Blackboard:
a PDF file of your presentation
slides
a web link to your
screencast presentation
a class discussion
question (see Part 2 for details on what this question is)
Include an
introductory slide with your name and title of your presentation.
Do not read directly
from your slides. Include short bullets on slides and expand on them in
what you say.
All information in
your presentation that is quoted or paraphrased from another source must
be cited correctly with in-text citations and on a references slide (you
do not need to say all of the citations during your spoken presentation)
in APA style. You must use at least five scholarly sources other than
the course textbook, which should be included on a references slide at the
end of your presentation.
Please submit the
presentation as both a PDF file of your slides (save your presentation
file as a PDF) along with the web link to your actual “live”
presentation. Large PowerPoint files that have not been converted to a PDF
will not be accepted. Your presentation slides must be converted to a PDF
file and submitted as a document. Converting your slides to a PDF will not
save your presentation/screencast in the file. Your “live”
presentation/screencast should be submitted in the form of a URL link
(e.g. a screencast website or YouTube/Vimeo). Please do not submit MP4 or
other large media files as your screencast. The link to your
presentation and your discussion question will also be posted to BB
for your classmates to view and comment on next week (see PHP
part 2).
