Vulnerable Populations For Community Health Nursing

Choose a vulnerable population from the chapters youve read on this topic (violence and human abuse, substance abuse, migrant workers, and poverty & homelessness).
Write a 1,000-word paper (not counting the title and reference pages) using APA format that encompasses the discussion topics and the answers to the questions below:
Discuss your community:
The name, geographical location, size (population), and climate.
Socioeconomics (median income)
Analysis of population by race, sex, age
Top 5 causes of death in your community compared to state &/or national data
Discuss the vulnerable population you have chosen as it pertains to the community in which you live:
Define and describe the vulnerable population
Examine health statistics and demographic data for your community:
What percentage of the communitys population does the specific vulnerable population represent?
Are there more males than females? Show percentage of sexes affected.
What ethnic groups constitute the vulnerable population? List in order of most to least in number.
Are many children affected? What are the ages?
What are the precipitating factors (the Social Determinants of Health) (video is close-captioned) that lead to one becoming a member of this vulnerable population?
What are the health issues common to this vulnerable population?
Outside of healthcare, what are the major life challenges for them?
Use the Community as Partner model (discussed in Chapter 17) to conduct a brief assessment of your community for services that focus on this particular vulnerable population.
Discuss the services/resources you discovered in each of the 9 categories (or as many that are applicable) for this vulnerable population.
Discuss the lack of services/resources that you discovered. This could be that resources exist but not in enough numbers or it could be that they dont exist at all.
Discuss any challenges/barriers that exist to services and use of resources in the categories from the Community as Partner model that were addressed.
How can these challenges/barriers be overcome?
Do a brief literature search to identify the latest, state-of-the-art interventions that have helped your chosen vulnerable population overcome challenges/barriers in other cities.
List three of these interventions that do not already exist in your community.
Identify one state-of-the-art but realistic intervention that you would be able to realistically assist in the creation and implementation of in your community. Explain how you would be able to assist with both the creation and implementation processes.
All questions in the outline must be answered for full credit. I have attached the class textbook where information can found and utilized for sources in this paper.

Avoids intrusion or judgment
