Psychiatric notes are a way
to reflect on your practicum experiences and connect the experiences to the
learning you gain from your weekly Learning Resources. Focused SOAP notes, such
as the ones required in this practicum course, are often used in clinical
settings to document patient care.
For this
Assignment, you will document information about a patient that you examined
during the last 4 weeks, using the Focused SOAP Note Template provided. You
will then use this note to develop and record a case presentation for this
patient.
Select an adult
patient that you examined during the last 4 weeks who presented
with a disorder other than the disorder present in your Week 3
Case Presentation. (Disorder selected is Schizophrenia)
Create a Focused SOAP Note
on this patient using the template provided in the Learning Resources.
There is also a completed Focused SOAP Note Exemplar provided to serve as
a guide to assignment expectations.

Include at least five
scholarly resources to support your assessment, diagnosis, and treatment
planning.

Present the full complex
case study. Include chief complaint; history of present illness; any
pertinent past psychiatric, substance use, medical, social, family
history; most recent mental status exam; current psychiatric diagnosis
including differentials that were ruled out; and plan for treatment and
management.
Report normal diagnostic
results as the name of the test and “normal” (rather than specific value).
Abnormal results should be reported as a specific value.

Subjective: What details did the
patient provide regarding their chief complaint and symptomology to
derive your differential diagnosis? What is the duration and severity of
their symptoms? How are their symptoms impacting their functioning in
life?
Objective: What observations did you
make during the psychiatric assessment?
Assessment: Discuss patient
mental status examination results. What were your differential diagnoses?
Provide a minimum of three possible diagnoses and why you chose them.
List them from highest priority to lowest priority. What was your primary
diagnosis, and why? Describe how your primary diagnosis aligns
with DSM-5 diagnostic criteria and supported by the
patient’s symptoms.
Plan: What was your plan
for psychotherapy? What was your plan for treatment and management,
including alternative therapies? Include pharmacologic and
nonpharmacologic treatments, alternative therapies, and follow-up
parameters, as well as a rationale for this treatment and management
plan. Be sure to include at least one health promotion activity and
one patient education strategy.
Reflection notes: What would you do
differently with this patient if you could conduct the session
over? If you are able to follow up with your patient, explain
whether these interventions were successful and why or why not. If you
were not able to conduct a follow up, discuss what your next intervention
would be.

For this assignment, you are to fill out the Hominin Toolkit Worksheet

with information about each of the different toolkits we will be studying in this module and the Module 6. The information about the tool types should come from our Explorations textbook and our course module pages. Any information required by the assignment that is not provided in the textbook or in our module pages you are free to find using the internet, but you should start with the information provided in our class. This assignment is your opportunity to apply
your learning about positive psychology using relevant research and theoretical
literature gleaned from the text/class lectures, and 3-5 scholarly research
articles (no more than 10 years old). you will create a presentation using
PowerPoint (or Google slides). For this presentation you will pick a topic that
interests you about Positive Psychology either on a personal level
involving how positive psychology can be applied to one’s personal life; or,
you may choose to do a presentation from a professional standpoint
on how Positive Psychology could be useful professionally (for example, in a
chosen career). Complete your presentation with a recorded Zoom (screen-sharing
your presentation/video on) and submit the link in the corresponding
assignment section in Canvas.
Here are some example topics if you choose to take the personal
application approach in applying positive psychology:

of your writing (typo-free), appropriate level of depth and overall clarity

Introduction
The use of observation techniques is an important tool in the belt of a marketing research.
Directions
In this assignment you will design an experiment using one of the observation techniques as your method of measurement. Include the purpose of the study, the question, and the sample choice. Write a small research group in the beginning which has grown since freshman year of college where we had several information about the heritability of diseases and disorders. I was in charge of looking through ICD-11 sources to find the common, broad and unique environmental changes and how they could be heritable. I need an essay over my research that has to explain how I was part of a group for a big project and the details and good explanations of what I leearned from it as a person. I need it to sound like I will be published for which I will be an author for and that team work is important. I have more information relating to this project that I will further explain: I need for you to MUST include the following things for this paper.

I can also give you a draft if needed that I have written which needs majority of help with the reflection part as I did not write that. It will be pointless however because I think it needs a fresh start since I was focusing too much on the details of the research I do not believe I would choose my own paper by the lack of flow.

I need you to write about 65% of
the text and rephrase and summarize what I have I provided while adding further
information to the other 35% like you already provided. It has to sound like I
know how to do research and basic database that helped me learn from the terms
and what I learned from it all basically.

Nonetheless yes you do need to know exactly what
the research position is about and the importance of it. The details provided
can sum up to become variable graphs (we created) that the team built on and
reflect from to show relations of the diseases and disorders.

It has to show how I know that I learned some broad
different ranges of diseases cause similar symptoms, I can now identify risk
factors, estimate risk of the diseases that could be passed on (heritability),
some treatment options for different environmental groups using the ACE
model (like A is unique so solely that person, additive genetic
variance, C is common or shared factors so a sibling/relative with it, and E is
specific/ nonshared environmental factors with errors calculated from the
global population to try to see the similarity in all of them as a whole at the
end).

The hypothesis of our research is that the diseases
in the same ICD chapter would have similar mortality (and even prevalence
rates) that would create a similarity in their heritability. It was proven
wrong of course because the chapters had to relation to the severity of some
diseases while others were minor enough to not be passed as much. It also
showed me that the chapters are for the bodily systems as a whole for each
significant system and does not account for the deaths they can cause (i.e like
we did not use any cancers because we know they would cause deaths higher than
others so they were ruled out, I found similarities in mortality in some
diseases in other groups, I used images to visualize the designs and learn from
it, incidences could be resolved with treatment but was not always the case
with certain diseases, etc.) The point was to analyze and try to find a
connection/reason for how genetics have an effect on disease heritability in
overall senses.
