If you are not currently employed in health care, you can select an issue at a former employer.
In this course, you will use the Model for Improvement, a four-step approach to quality improvement, which is based on the Institute for Healthcare Improvement’s (IHI) recommended model. The four steps of this approach consist of identify, analyze, develop, and test and implement.

Each week you will work on one of the steps of quality improvement. This week, you will complete Step 1, which is to identify the problem.
The first step is to complete a review of the workplace to identify a problem.
Consider your daily work. What areas of concern do you observe happening that have a negative impact on patients in your organization?
To determine whether the problem is appropriate for a quality improvement (QI) initiative, consider the following questions:
What is the problem?
How do you know that it is a problem?
How frequently does it occur, or how long has it existed?
What are the effects of this problem on stakeholders and on the budget?
How will we know when it is resolved?
Write a 250-word problem statement that succinctly defines the problem.
Massoud et al. state:
[A problem statement] helps to clarify and communicate the area identified for improvement. A problem statement is a concise descriiption of a process in need of improvement, its boundaries, the general area of concern where QI should begin, and why work on the improvement is a priority. In creating a problem statement, it is important to avoid listing potential causes or solutions, and to focus energies on describing the problem. It is also important to note that problem statements should be carefully constructed to not assign blame to a particular person or department. The assignment of blame not only makes assumptions about the cause of a problem, but also alienates key people from the design and implementation of solutions (2001).
Write a 700-word paper answering the 5 questions for QI initiatives from above, as well as the following questions:
Is there measurable data that can be used to demonstrate the problem’s existence?
What impact does that problem have on the 6 dimensions of quality?
What regulatory agencies are involved that are driving the need for improvements? What is their standard of quality regarding the problem?
For example, The Joint Commission (TJC) publishes its National Patient Safety Goals (NPSG) each year, which are meant to help organizations address issues of patient safety. One of the NPSGs is wrong site procedures. This includes a list of elements that must be included in the work processes to ensure that wrong site surgeries/procedures do not happen. If these untoward occurrences do happen, the event is reportable to TJC. Further review and action plans are expected as the organization works with TJC toward resolution of the quality concern/event.
