Accident Causation and Investigation Consulting Report
Unit VI Project
Accident Causation and Investigation
Prepare a project report using the attached Unit VI Consulting Report template (in the attached documents) for your project, and complete the template based on the following information:
Part I: List the five most significant financial benefits for an organization to invest in a safety system or program.
Part II: Acme Construction is due for a workers’ compensation premium review and you have been asked to estimate the potential premiums for the coming year. You have 10 masonry employees (classification—5022) with a rate of $7.96; four plumbers (classification—5183) with a rate of $3.86); and two electricians (classification—5190) with a rate of $4.32. The total payroll for the three classifications are as follows:
5022 masonry $416,000 (total annual payroll)
5183 plumbers $208,000 (total annual payroll)
5190 electricians $145,600 (total annual payroll)
The previous experience modification rate (EMR) was 1.0 because the company had only been in business for 3 years and the EMR is based on a 3-year rolling average. In those previous 3 years, your company had zero claims and you can anticipate that your company will have a reduction in the EMR to at least 0.90. However, you have not received notification of this new EMR, so base your overall premium calculations on an EMR of 1.0
Part III: Using the results from the 1.0 EMR premium estimate, calculate the rate to one of a 0.9 EMR and then a EMR 1.25 rating. Compare both of them to the previous EMR of 1.0 and show in a table as was depicted in the unit lesson.
Part IV: Provide a summary of the energy release theory of accident causation.
Part V: Explain the near-miss relationship as referenced in Figure 20.7 of your textbook in your own words.
Ensure you address all parts of the template. Your project should be at least three pages in length using the above template. Make sure you show your equations and all work for determining answers for Parts II and Parts III in the project template.
No references are required for this assignment; however, if outside sources are used, please adhere to APA Style when creating citations and references for this assignment.

Ethical Application
First, choose an issue in applied ethics that greatly interests you. You may choose any of the following topics covered in the assigned readings from our textbooks:
For example: Abortion; Biomedical Issues (IVF, Stem Cell Research, etc.); Capital Punishment; Church-State Relations; Economics; Environment; Euthanasia; Just War; LGBTQ Issues; Marriage & Family; Healthcare; Political Engagement; Poverty; Racism and Race Issues; Sexual Ethics, etc.
Second, based on the ethical theory you defended in Discussion: Compare and Contrast Metaethical Theories, formulate an ethical application on this specific issue in a thread. You are expected to use both textbooks as well as outside academic sources to support your analysis and argument. As you write your thread, consider the following flow for your essay:
Step 1: Briefly summarize what theory you defended in Discussion: Compare and Contrast Metaethical Theories and what applied issue you are addressing in this discussion. If you have changed your mind on which theory from Discussion: Compare and Contrast Metaethical Theories you think is stronger, you may write an application for the theory you NOW think is the stronger theory. For example, if you defended Egoism and now think a Christian ethical theory is stronger, you may apply a Christian ethical theory.
Step 2: Articulate a detailed application based on your theory. This should be the bulk of your thread.
Step 3: Briefly describe a strong counterargument to your application and respond to it. For now, keep it brief (no more than one paragraph). In the Capstone Essay Assignment, you will have more room to elaborate.
Memo on affirmative action
Suppose you work for a major hotel chain in a human resources leadership capacity. One day,
your boss asks you to dra a memo for the company’s leadership team regarding the propriety
of affirmave acon in hiring. The company is concerned that its employee base isn’t diverse
enough, and that they risk scruny and perhaps even disparate impact discriminaon penales if
their workforce demographics were to ever be audited by the government.
In no less than 2,000 words, take a stance on the issue of affirmave acon, and arculate your
opinion to your company’s leaders in a professional memo. Although you will be defending one
side of this issue (either for or against), be sure to analyze and discuss BOTH sides. Your
submission should include, but is not limited to, thoughts on the following quesons:
1. What are the arguments in favor of each side?
2. What are the arguments against each side?
3. What relevant case law exists on the subject of affirmave acon? Which side does it
favor?
4. What is the EEOC’s current posion on voluntary workplace affirmave acon policies?
5. What other companies in the industry have adopted affirmave acon policies? To
which minories do they give preference?
6. What potenal consequences would there be if your company’s leaders follow your
advice?
4
7. What potenal consequences would there be if your company’s leaders ignore your
advice?
Remember that this project is a legal analysis of facts and circumstances, not an editorial piece.
In the law, you don’t get to say something ought to be a certain way because “that’s the way I
feel”. You must defend your posion with sound, raonal reasoning. As such, research is an
integral part of this project. Your memo should include a minimum of 5 external sources, and
they must be cited properly in APA format.
This is copy and pasted straight from the syllabus.
Applying an ethical decision-making model (see instructions for questions)
Most often we encounter ethical dilemmas related to end-of-life issues. The purpose of this forum is to practice applying the 6.0 Model of Ethical Reasoning PHI 1600 Applied Ethics Dr. Dave Monroe (attached). This decision framework was introduced in Week 1. You will apply it to an ethical dilemma you encountered, or have personal knowledge of, in a nursing setting. About 90-100 words per answer to each question.
Confidentiality is important! Do not disclose names of patients, staff, or agencies as you describe an ethical dilemma witnessed. Remember, an ethical dilemma is a problem that has multiple & often opposite but ethically justifiable solutions. Most of the time there is no “perfect” solution, but a reason-based decision must be made.
1. Briefly describe the ethical dilemma in nursing with necessary background information. Include a description of the major stake-holders. Who held the “power” or control over the final decision?
2. What were the options considered for resolving this issue? Were there specific regulations or agency policies applicable in the issue?
3. Which provision or specific interpretive statement of the ANA Code of Ethics for Nursing (attached) is relevant to the situation you described?
4. Discuss whether you think the “best” moral decision was selected. Is this an instance that contributes to ‘moral debris’ because of what was not done or could have been handled better?
