The identification and implementation of evidence-based practice must first begin with an assessment of current state and the recognition of areas in need of improvement. The Institute of Medicines definition of quality healthcare in their report Clinical practice guidelines; Directions for a new program is the degree to which health services for individuals and populations increase the likelihood of desired health outcomes and are consistent with current professional knowledge (IOM, 1990). The concept of current professional knowledge is inclusive of acknowledging both actual and potential quality issues, and this can be successfully accomplished through the reporting of variances in outcomes, clinical misadventures, and healthcare disparities. There are multiple organizations with a focus of knowledge-based healthcare delivery at the forefront of their scope and practice, and their influence can be selective or comprehensives depending on the needs of the audience. The over-arching goal of these entities is to support proposals based on the most current research of health-related issues, identify best practices, and facilitate change implementation. For the intent of this discussion post I will highlight specific contributions of the Institute of Medicine (IOM), the Agency for Healthcare Research and Quality (AHRQ), The Joint Commission, and their respective contributions to todays healthcare environment.
The IOM, now called the National Academy of Medicine (NAM), was founded in 1970 as part of The National Academies of Science. Their mission and vision centers around health equality, collaboration, and research-based solutions. In addition to publications on quality, clinical decision making, and healthcare equity, NAM routinely releases perspectives on current national and global health and quality issues for public viewing (https://nam.edu). One of the most widely recognized reports published by this organization is the 2021 Crossing the quality chasm: A new health system for the 21st century (IOM, 2001). This report was pivotal in that it brought to light many widespread defects in the then state of care delivery such as a lack of quality, cost concerns, inefficient use of technology, lack of progress for system restructuring, and the underutilization of resources (Murphy, Staffileno, & Foreman, 2018). It was discovered in these findings that research results were not equating to evidence-based practice which led to many organizations revamping reporting metrics and quality initiatives. Performance expectations were outlined, rules to guide patient-clinician communications were delineated, a framework was offered to better align quality improvements and payment incentives, and steps to achieve evidence-based practice implementation was provided (IOM, 2001).
The Agency for Healthcare Research and Quality (AHRQ) was established in 2003 in partnership with the U.S. Department of Health and Human Services (DHHS). This organization publishes the National Healthcare Quality and Disparities Report (QDR) to identify the national areas in need of most improvement with a focus on the evolution of quality care over time. The framework of this report includes a focus on better care, healthy individuals and communities, and affordability of healthcare services. (Curley, 2020). The AHRQ also designed specific patient and facility quality indicators. These indicators help acute care facilities in identifying the occurrence of often preventable conditions impacting morbidity, mortality, patient safety, complications, and hospital-acquired conditions through the monitoring and tracking of metrics such as prevention quality indicators (PQIs), patient safety indicators (PSIs), and pediatrics quality indicators (PDIs) (Agency for Healthcare Research and Quality, 2017). The AHRQ influences knowledge-based healthcare delivery through the identification and reporting of patient occurrences in an effort to bring awareness to areas in need of change. Evaluation of facility specific, patient specific, or population specific occurrences and/or outcomes can help organizations recognize areas in need of EBP implementation, may highlight breaks within current processes, and ideally identifies areas where methods are not working as intended. This further promotes quality efforts and the use of evidence-based practices and clinical practice guidelines.
The Joint Commission is a national agency who has at its foundation the aim of improving the quality and safety of health care with a goal of zero harm (https://jointcommission.org/). Each year National Patient Safety Goals are released for public review which are based on previous year(s) reporting of sentinel events, system processes, performance measures, and emerging hospital projects. For 2021 the patient safety goals for hospitals include the correct identification of patients, medication safety, medical equipment alarm safety, infection prevention, the identification of patient suicide risk, and preventing surgical mistakes (2021 Hospital Patient Safety Goals, 2021). The identification of key metrics for current national focus facilitates hospital efforts in addressing known safety concerns and allows for the evaluation of current practices and adjustments as needed to improve safety and quality overall.
