Mary is a 26 year old obese woman with a high school education and a job as a cashier at Home Depot. She works nights so her mother can help care for her children. Her mother has advanced Type 2 DM and is unable to walk or stand well. Mary is the single mother of 4 children (ages 8, 6, 2 and 2 months). Her children have 2 different fathers. She does not receive any child support or alimony from her ex boyfriend (the father of her first 2 children). Currently, she is seeing a nice man, who also has limited education and income. He is the father of her two youngest children. Mary’s first child has ADHD, her second child has Type 1 DM, but the other two are very healthy. Mary was recently told she has Type 2 DM and will need expensive medication daily.
What factors will affect Mary’s ability to care for her children in response to their health care needs? How can this affect her job and health insurance? How precarious is her ability to care for her family now that she, too, has diabetes? What will happen if she loses her job and health insurance? What if she becomes unable to work due to her or her mother’s health issues?

Students should identify a transitional care problem and correlate the
implications of the problem to a population and setting of interest. Review the

findings. Summarize and synthesize findings as they relate to patient
safety.

Population Health
Healthcare today faces many challenges, not the least of which is access to care. This is evident by the fact that in the patient-centered medical homes, a transitional care model, access to care is a “must-pass” component when seeking accreditation. However, to provide access to care, we must have adequate providers, which is becoming a challenge. In fact, the Association of American Medical Colleges (AAMC) estimates a projected shortage of medical doctors ranging from 61,700 to 94,700 within the next decade (www.aamc.org

). And while nurse practitioners are appropriately trained to fill the void, we have our own practice struggles limiting our ability to deliver the care we are trained to deliver.
In addition to the shortage of medical providers, few would argue that our system is broken because there is little continuity of care, poor communication, and duplication of testing, causing high costs and inefficient follow-up.
