What is needed to gather the data:
– Patient Clinical Notes
– Diagnosis and Clinical from physician
– Clinical from nurse
– Tickler from Billing department with list of patients cases that were sent back from claims
department for not paying out due to errors
– Research patient medical history
Who is involved:
– Physician (w/o clinical from physician then we would not be able to process billing and
send over to claims for a payment to be made to the hospital and/or doctor for services
performed. Patient would not have any medical records)
– Nurse (w/o clinical from nurse there would be no medical records or updats on patient
health or treatment)
– Outpatient Department (w/o clinical then patient would not be able to get physician or
nure professional opinion on their care or treatment.
– Health Information Management (w/o clinical medical record staff would not be able to analyze records for accuracy, review quality of records, Transcription and distribution of histories, discharge summaries, progress reports, consultations, and other medical records, Releasing patient information and protections for drug treatment, closed-adoption, alcohol
treatment, sexual and behavior health issues
– Medical Coder (w/o clinical medical coder can not code for research, reimbursement.)
Things that would be helpful to get the data:
– Query for clarification and specifics (This system goes in detail)
– CDI (Clinical Documentation Improvement) – process used by healthcare providers to
review clinical documentation and provide feedback to physicians that improve that
documentation.
