Dental periodontal case study

I am wanting a three page case study paper describing:
-What’s a dental periodontal case study is
-what is periodontal disease and how does it affect a persons oral health.
-describe the different ASA classifications, but go into detail about ASA 2 ( This is what the patient will have)
-describe the different periodontal staging and grading, but go into detail about stage 2 grade b ( This is what the patient will have)
-explain how smoking affects the oral health.
– describe a patient having a class 4 heavy calculus
and being able to see radiographic calculus on dental radiograps.
-explain what a scaling and root planning is (Srp) and how this type of cleaning can help a stage 2 grade b peridontits patient.
-in conclusion, explain the importance of getting routine, dental cleanings and helping periodontal patients. And include a success Story of a patient with stage two grade b periodontal disease having a positive response to the SRP deep cleaning and the importance of returning for periodontal maintenance.
