Research and read 3 nursing articles related to the chosen topic and the assigned Master’s Essential – the articles must be cited within the paper.

Describe a scenario where you are part of an interprofessional team that is trying to improve processes related to one CMS core measure, such as for heart failure.
Identify a real or virtual patient care error, perform an analysis of the causes of the error, and identify quality improvement strategies to address the identified cause(s).
