Simply gather information about your family’s current health, if living, what diseases they have or have had, what treatments they had if any, how they are dealing with any major illnesses, their major symptoms, list birth defects, still births, etc.. If the family member has passed away, write down the cause of death, age, and any health information you can gather about them. You can include the person’s diet, lifestyle, exercise or lack of exercise, etc, and include habits, good or bad.
If you want certain things to remain private you can simply write them down for yourself. You do not have to share or go into detail about private matters in your summary that will be shared with the class.

Based on what you know about your family’s health history, your diet and lifestyle habits (exercise, drinking, smoking, etc..) what ailments will you most likely be faced with in the future? What should you be watching out for? Does something tend to run in the family? What are you doing to avoid it?
What SHOULD you do to avoid it? How can you change your diet and lifestyle to decrease your chances of it? What are you concerns, fears, and/or hopes for your health future?
It’s time to put on your thinking caps! Something is out to GET YOU! What is it? Why? How? Whats your escape plan? Investigate this fully and in detail.. your LIFE depends on it!
Part Two–> This is the only part of the Family Health History that you will turn in. Post a summary of your findings, leaving out private information, in 500 words or more. Base your report on what you have learned, what your family health history means to you, what you might acquire when you get older, how can you prevent or improve your chances of avoiding the illnesses in your family history, and give details about the improvements so that you can avoid such illnesses. (Please do not simply state that you need to eat more vegetables.. If that is the case then list vegetables you will be eating more of and why.) I want to know you have done the assignment and have learned something from it, including what to do to improve your health in detail.
