Scenario: You have two patients on your unit. They are as follows-
Patient 1: Ms. Apple is a 26yr old female admitted for liver failure. Pt is jaundice and cachectic. She has an extensive alcohol and substance abuse hx leading to Hepatitis leading to pts liver failure. Both parents and sister are at her bedside. Her labs are looking grim and is in need of a liver transplant. Pt is confused and in pain.
Patient 2: Mrs. Plum is a 66yr old female admitted for liver failure. Pt is jaundice and cachectic. Her liver failure is due to Autoimmune Hepatitis. Husband and 2 children are at the bedside. Pt is expecting her 1 st grandchild in the next 3 months. Her labs are looking grim and is in need of a liver transplant. Pt is confused and in pain.
One liver has just become available. Both patients are the same blood types, and both are found to be a match for this liver. Dr. S must decide who gets the liver.
Ethical dilemmas like the above scenario happen every day. What ethical challenges arise in this type of scenario? How do you choose without bias? Who gets the liver and why?
