Develop a report that reviews the three case studies and describe the findings. I would also like you to answer some questions within the body of the report that pertains to the primary modes of failure and a Root Cause Analysis. You need to:
a) summarize the primary mode or modes of failure described in the case studies that your group reviewed and clearly reference the case studies in your bibliography giving credit to their findings.
b) Distinguish the methods used by the investigators in your chosen case study to determine/identify the root cause of failure.
c) Specifically answer the question: Did any of the authors use a Fault Tree Analysis Diagram and a Failure Mode Assessment Chart to distinguish the root cause? If they did, comment on whether you feel their analyses were rigorous and complete. If not, that is something you will need to develop and include in your report. Comment on what your fault tree says is the likely root cause and if that was examined adequately in the reported study.
