Listen to the interview in its entirety, taking notes with regard to the following issues to formulate your post:

Doctors exist within a culture of secrecy surrounding medical mistakes. Yet doctors are not the only source of mistakes within the medical community. Should other medical professionals such as nurses and/or medical technicians admit their mistakes to their patients? What procedures should be in place to provide support for medical professionals who make a mistake? What are some structural changes that could potentially lower the likelihood of error? What challenges does Dr. Ofri identify in reporting and/or documenting medical mistakes and near-misses–and how, in your opinion, could these instances be more effectively addressed?
