Case Study: Reducing Readmissions for Heart Attack Patients
Nihar Desai, MD, MPH, a cardiology fellow at Brigham and Women’s Hospital (BWH), and Jeff Durney, previously a project specialist in the quality and safety group at Partners who now works as a performance improvement consultant at BWH, participated in CPIP in the Fall of 2011. The pair focused on reducing readmissions for patients who have suffered heart attacks. One out of every seven patients who experiences a heart attack will wind up back in the hospital within 30 days, and many of these readmissions are preventable.
