Delirium in an acute hospital is prevalent in hospitalized older adults, with a rate of
21-22% in medical and orthopedic conditions, with dementia as the significant
risk factor for Delirium (De et al., 2017).
Low detection of Delirium can result in complications, prolonged
hospitalization, falls, institutionalization, or death (De et al., 2017). It is a quality
improvement (QI) project using an intervention and post-test design conducted
in the surgical unit of a 200-bed acute care hospital in North Texas. At
present, there is no standard delirium tool being used at the hospital. This
hospital is a Level II Trauma Center, accredited Chest Pain Center, and a
Primary Stroke Center. This project seeks to increase delirium assessment and
identification of Delirium in a hospitalized older adult in an acute surgical
unit. Nurses perform the 4AT delirium screening tool. We will use the QI
methodology to conduct the project using the Plan Do Study Act (PDSA)
framework. Statistical analysis will be
performed using SPSS software, and descriptive statistics will be performed on
demographic variables such as gender, age, comorbidities, and frequency and
severity of adverse effects.

14 hours ( from October 10, 2021 to October 17, 2021) – Education to nurses during day shift and night shift huddle meeting on how to assess delirium in patients during daily shift assessment. Nurses are educated on Delirium pathway and PINCHES ME.
PINCHES ME is a useful mnemonic for the review of possible causes for delirium :
P – Pain
I – Infection
N – Nutrition
C – Constipation
H – Hydration
E – Endocrine + Electrolyte
S – Stroke
M – medication and Alcohol
E – Environmental

– Learn how to identify strategies effectively assess in older adults and how to use nonpharmacologic options for pain management in older adults.
