Student task groups will develop a written intervention proposal that describes a proposed program or change in an organization. The intervention proposal should be double-spaced (unless otherwise noted, such as in tables, where single-spacing is permitted), and approximate length of 25 pages, including references and appendices. The proposal should be visually appealing and appropriate for a professional audience. The completed proposal should be submitted on Canvas. The completed proposal should include the following components:
Title Page (1 page)
Table of Contents (1 page)
Executive Summary (1-2 pages)
Stand-alone overview of entire program plan, including resource need
Brief rationale for the program
Includes brief program description, number served, and expected outcomes
Significance & Rationale (2-3 pages)
Clear statement of problem and its significance
Convincing rationale for why program is needed (grounded in literature, theory, research, and social work values)
Intervention Hypothesis, Goals, Objectives & Activities (1-3 pages in outline format)
Intervention Hypothesis

Logic model
Program Design & Structure (3-4 pages)
Description of design and structure
Convincing argument as to why your design/structure is creative and relevant

List of data elements to be collected in MIS (list can be single-spaced and in columns)
Fit of MIS to objectives

Potential funding and sustainability
