ASSIGNMENT 1, PART A: DISEASE/ILLNESS/CONDITION PATIENT EDUCATION HANDOUT
For Part A of this assignment, you will provide a descriptive summary of a health issue or disease/illness/condition describing the medical aspects of the disease, risk factors, treatment options, and prognosis.

Steps to Complete This Assignment
1. Write a two-page patient handout. Your handout should be creative, as if you are preparing it for patients and their family or a community. Be sure to include illustrations, diagrams, photos, or graphs. The summary should contain these headings:
• Description of the health issue or disease—main features, symptoms, and characteristics.(15 points)
• Population(s) affected by this health issue. Include age, gender, race or other demographics. Address prevalence, risks, and severity, along with any preventative measures. (15 points)
• Describe the method by which this disease is best treated (per evidence from scientific literature). Be specific. (15 points)
• Explain the ways the illness/disease may impact the individual and their family. Include any psychological, social, or emotional issues that arise from the illness or treatment and best emotional or psychological methods to improve and maintain positive mental health. (15 points)
• Highlight one community/local, state, or federal resource that may assist the patient or family with additional information, financial assistance, referrals, or emotional support to deal with the impact of the disease. Include how to access the resource and describe specific services offered that may be supportive for your client. (15 points)
• Creativity, graphics, grammar, organization. Include your references on a separate attachment. (25 points)
