Purpose
This assignment will help you become familiar with an
evidence-based program and evaluate the program by applying the Spectrum of
Prevention Framework.
To complete this assignment, you will need to:
1. Identify an evidence-based health care system
or community-based intervention/program designed to prevent or control a chronic
disease.
2. Briefly describe the intervention/program and
identify the outcome of interest (for example, increased cancer screening,
improved high blood pressure) and the target population.
3. Apply the Spectrum of Prevention framework to:

o Identify which levels of the framework the
evidence-based program addressed.
o Identify and describe the different
intervention components (and/or strategies) that fall within each level in the
framework that you identified above.
o Identify which levels of the framework were
not addressed by your evidence-based program.
o Identify and describe one or two possible
strategies for each of the levels identified above.
