Prevalence: Provide prevalence rates in national terms and then state using appropriate references.

Contributing Behaviors: Identify and describe specific contributing behaviors (actions) that may cause or worsen the health problem using appropriate references.
Priority Population: Identify the specific priority population including their characteristics, predisposing, enabling and reinforcing factors.
Show relationship to priority population by answering “why is this a problem?” and “why should it be addressed?”
Other Programs: Identify and discuss other programs that have addressed this same issue (describe their program overview, strategies, and outcomes) using peer reviewed, scientific references.
Guiding Theory: Identify and describe the appropriate behavior change theory that you will use. Identify the specific constructs you will use from the theory to select your program (intervention) strategies.
Program Overview: In a program overview, state your proposed solution to the problem (your program, project, or campaign) including who, what, when, and where it will take place. Defend why it will be likely to work using peer reviewed, scientific references (identify the SPECIFIC BEHAVIOR you will intervene on in YOUR program).
Bibliography/References: Include at least 3 peer reviewed journal articles.
Strategic Plan: Include your strategic plan addressing the mission, goals, objectives, strategies, and evaluations (behavioral, cognitive, affective) for your program. Objectives should follow ABCD format and be specific.
Appendices: Include a self-evaluation of your paper using the Program Rationale Rubric.
Rationale and references should adhere to APA format (Times New Roman, 12-point, double spaced, 1-inch margins). At least one strong paragraph (at least 4 sentences) should follow each of the headings (indicated by the numbers above).
