Identify the key components of one behavior change model and explain how you will use this model to assess Sue’s problem/s.

Sue has been referred to you since you have expertise working with troubled adolescents. She is 15 years old. She is rumored to be engaging in oral sex with multiple boys in the school and she is developing a negative reputation among her classmates. She comes from a middle-class family. Her parents recently divorced. She is a smart student, but her academic performance has declined over the past year. She feels that she does not fit in with the other girls and her weight gain over the past year has affected her self-esteem. She has come to see you because she recognizes she has a problem and wants you to help her.

Develop a six-week health counseling intervention plan to help Sue (7-10 pages). Your plan should include the following:

b) Describe how you will determine priorities, set goals, collect data, infer needs from data obtained, and measure behavior change.
c) Describe how you will assess the impact of your intervention on her behavior.

3) Describe counseling strategies that you will use to help her.
a) Briefly describe five counseling skills that you feel will be most useful in helping Sue develop insight into her problem/s.

4) Discuss how you will collaborate with other professionals to assist Sue.
