IHWA Plan and Evaluation

create a Voice-Over PowerPoint presentation based on the IHWA assessment and analysis for the CPE project. Include the following in the presentation:
1. Introduce the person assessed and summarize IHWA findings (do not mention the person’s name or last name in presentation slides or speaker notes)
2. Identify one priority area to focus on improvement and include barriers and opportunities for improvement
3. Create a plan of care that includes: two holistic interventions that include coaching strategies, needed resources, and educational materials.
– The holistic interventions can be CATs or other interventions.
– Using SMART goals is suggested as you will be evaluating them later in the paper.
4. Incorporate at least one holistic theory
5. Include cultural/ethical/community impacts
6. Explain measurement strategies for the evaluation of the plan of care. By using SMART goals in the plan of care section, will be straightforward.

Ensure a title slide, smooth transitions between slides, and several slides listing the references used.
Use each numbered item above to separate your slides effectively.

The presentation should be 8-10 slides long (including title and reference slides).
