Course Project Description: The course project will require you to describe innovations (technological, policy, structural, procedural, etc.) to address one of the common elements of the chronic disease models related to a chronic pulmonary disease of your choosing. You may find examples of these innovations in publications such as the GOLD guidelines or the EPR-3, in institutional programs (your place of employment for instance), in resources from organizations dedicated to treating the disease (whether governmental, public, private, etc.), in the care practices of other chronic diseases (such as diabetes, CHF, etc.), and even in your imagination if you cannot find an existing example. Be sure to site and give credit to innovations you encounter. You must explain how the innovation relates to the common element and to the chronic disease upon which you are focusing. Your project should at least include the following sections: (1) Introduction in which chronic disease models are discussed in general and where you list and explain the common components. (2) Overview of the chronic disease you are addressing. (3) Meat of the project where you address the project’s requirements. (4) A conclusion summarizing the project or integrating the components.
