part 1: Leadership Portfolio
This assignment will allow you to plan and implement a Play Day for a school or a Community Event (approved by your instructor). In a group you will plan one event, but will be participating in all events. At the end of the assignment you are to hand in all your work.
The first page of your assignment should be a title page that includes your name, the course, instructor’s name (spelled correctly), an appropriate title, and the due date. ___________ Feel free to decorate your title page to make it reflective of who you are.
The second page of your assignment should be a table of contents that outlines what will be included in your assignment with appropriate page numbers and sections.
The last three pages of your assignment should be the evaluation checklist sheet and the assignment rubrics that are a part of this assignment sheet.
Play Day at a school or Community Event
As a class, you will be responsible for hosting your last major event of the year. This will be in the form of a “Play Day”. You will be responsible for producing the following components:

Part 1: Smart Goals (Group) /6
With your group members, you will be required to come up with 2 goals that the group will aim to achieve during the event. You personally, will come up with 1 personal goal. Your personal goal should be based on something you have learned in the course with regard to leadership and recreation. This could include leadership style, personality, communication etc. Make sure to follow the SMART principle as much as you can. Please document when you will be meeting as a group either via adobe connect, skype or google hangout. (Record the meeting as a way of documenting what you have done)
Part 2: Brainstorm (Individual from group info.) /6
Although the group will come up with the final copy of an action plan (see below), each individual student must hand in their own rough work from brainstorming that must be done ahead of your group meeting. This brainstorming must be written on the appropriate handout and include three sections: themes, considerations, activities.
Part 3: Action Plan (Group) /15
Before the event, specifically on ___________ , you will have to submit your ACTION PLAN. The goal of the action plan will be to outline what you and your group members will be responsible for before the event, during the event, and after the event. You can structure the action plan in any way you see fit. Make sure it is organized and easy to understand. (see worksheet)
Part 4: Survey (Group/ Individual) /10
With your group, you must come up with a brief evaluation survey that contains 5 questions and will be handed out to the participants of the event. You may frame the questions as you did for your surveys. Make them age appropriate, short and specific. This should only take participants about 2 minutes to fill out. This will be important since you may use the feedback from the participants for Part 5.
ONE PER GROUP
You will compile the data as a group and create a BRIEF summary. This will include a maximum of one page with data and a conclusion that is to include one sentence about the strengths and one sentence about the “room for improvement” aspect of the day in direct relation to the participant’s surveys. You are to hand in all the completed surveys with the BRIEF summary attached to the top. ONE PER PERSON
Part 5: Reflection (Individual) /15
After the event has taken place, you will be required to individually submit a reflection on how you think the event went. Here are some possible topics that you should discuss in your reflection:
-What went well during the event? What didn’t?
-How did you take a leadership role before and during the event? What did you do?
-What characteristics of leadership did you exhibit?
-Did you and your group members meet your goals?
-Did you meet your goal?
**be specific and give examples where possible**
The reflection should be minimum one page typed, double spaced with size 12 font. Please no more than 3 pages. Use complete sentences and be clear and concise with your thoughts.
Part 6: Actual Play Day /20
You will be given a mark out of 20 for the play day. This is evaluated by the supervisor on the day. You will need to inform your teacher whom will be the supervisor that date.
Part 7: Rough notes/drafts/schedules /10 (journals 6 of this)
Keep all of your rough work and put it after your reflection. This includes any notes, drafts and schedules you personally complete in class time and outside of class AND journals.
