ASSESSMENT DETAILS
The aim of this assessment is for students to develop an understanding of continuing care for long term and chronic conditions to meet the Learning Outcomes for the module.

a) Discuss how the nurse can demonstrate cultural safety and cultural openness when providing long term care for the individual and their family/whānau.

b) Compare and contrast the two models of hauora Ayurvedic model and Meihana Model and explain how the nurse can utilise the Ayurvedic Model to support the individual with their long-term condition and their family/whānau.

Case study – ‘Sudevi Patel’ (Indian)
Sudevi (78-years old) had a stroke six (6) months ago and was moved to an aged care facility by her family. Before this, Sudevi lived with her husband Kumar and two of their five children in a suburb in Hamilton. Her 35-year-old daughter and 42-year-old son have their own families but share the family home.
Sudevi’s stroke occurred in the left hemisphere, in the cerebrum, which affected her mobility and her ability to communicate. She is not able to mobilise without support (she requires a walking frame and one person to assist) and has expressive aphasia. Sudevi has always been very proud of her ability to cook meals and pass on her Indian traditions in the home.

Sudevi’s health history includes:
● Stroke with subsequent hemiparesis and aphasia (six months ago)
● Arteriosclerosis and atrial fibrillation (managed with medication)
● Hypertension (controlled with medication)
