i have a concpet map due. I have pt. information as well as samples and refernces from the interenet that i will compile into documents submitted to you. admitting dx of SOB and hyperchloremia (“no appetite, no energy”-her words. onset was 2 weeks prior)

pt was given a swallow test which was failed a chest xray was also given at bedside showing no noticeable plueral aspiration

VITAL SIGNS: temp 97.4 tympanic pulse 85 Respiratory rate 18/minute O2 98 room air BP 132/60 lying down

ASSESSMENT: pt. was weak upon assessment with minimal strenght in lower extremities, pt. complianed of abdominal pain and difficulty in swallowing and finishing meals. pt was requesting PEG tube to supply proper nutrition. no IV drips presently running but IV is present in right AC and is clean, dry and intact. Pt. was admitted with shortness of breath and hyperchloremia.
