ASSESSMENT: pt. was weak upon assessment with minimal strenght in lower extremities, pt. complianed of abdominal pain and difficulty in swallowing and finishing meals. pt was requesting PEG tube to supply proper nutrition. no IV drips presently running but IV is present in right AC and is clean, dry and intact. Pt. was admitted with shortness of breath and hyperchloremia.
