You will submit a power point presentation to include the following components:
1. Problem Statement – what problem did you work on; what are you trying to improve? Why did you select this problem? What data do you have to support the problem?
2. National Context:
State how your problem is linked to a national quality or safety indicator for JC, NQF, NCQA, or Healthy People 2020; state which indicator is included in one or more of these databases that is similar or the same as yours.
State THAT your problem is linked to a national nurse-sensitive quality indicator, which one?
3. Background Information about the problem
Your quality framework should be briefly described. Summarize evidence-based articles relevant to your problem and clearly state how you used this information in your QI process. This section should include the identification of the QI Model. Identify the model (Refer Wyant (2017) utilize one of the commonly used models as a framework for your project); describe how the model is used to guide the process.
4. Narrative summary of expected outcomes.
The narrative should also include the following:
If your project is successful, what methods and specific plans do you have to maintain/and extend a successful project solution.
If your project is not successful, what methods and specific plans do you have to revise/and or terminate an unsuccessful project solution.
Describe specific plans for feedback in the work setting and for communicating the project and its results to professional groups and/or colleagues external to the project.
Describe barriers in the chosen setting and identify the plan to overcome.
5. Conclusions/ recommendations for practice, research and/or education
6. References- Include 4 evidenced-based current references from nursing/healthcare journals.
