Define health
literacy? is the degree to
which individuals have the ability to find, understand, and use information and
services to inform health-related decisions and actions for themselves and
others. When organizations or people create and give others health information
that is too difficult for them to understand, we create a health literacy
problem. When we expect them to figure out health services with many
unfamiliar, confusing, or even conflicting steps, we also create a health
literacy problem. Most recently, Healthy People 2030 (Links to an external
site.), which establishes national objectives for health
improvement, has included attaining health literacy to improve the health and
well-being of all as one of its five overarching goals. Healthy People
2030 also adopted two definitions that together constitute health
literacy

Health literacy is important in nursing
because it increases
patients outcomes. As nurses working alongside providers its
important that we educate patients. We can use a multitude of teaching methods.
But we must make sure were giving science-based teaching without dumbing down
the information or distorting the science. Using the 3 As will help nurse
provide education as well as assess patient health literacy.

Accurate: Using health literacy best practices
does not mean dumbing-down the information or distorting the science. Health
literacy practices make sure information is presented accurately AND in ways
that people can understand.

Accessible: Just because you create health
information doesnt mean people see it or can use it. Where and how you present
your information affects its accessibility. Key aspects to consider:
o Is the information where people can see it?
o How will people who arent actively looking
for your information be exposed to it?
o Do you have a main message statement?
o Have you made the information easy to skim and
scan with large font, sub-heads and bullets, and white space?
o Do the images match the text and have useful
captions?
Even in the digital age, where posting information on the Web is easy and
low-cost, the Web is not always the best way to reach the general public.
Multiple channels and formats are best, and it is your responsibility to ensure
the information you created reaches the public in a usable format.

Actionable: It is human nature to want to tell people all
we know about something, but that doesnt necessarily help. In the health
field, we typically want people to start or stop doing something or do more or
less of something. However, we often spend our resources giving people
background information instead of our recommendations. Some background
information may be important, but make sure you provide actionable information
so the people you want to reach CAN do something with the information provided.

Tools available to assess health literacy? AHRQ-funded researchers have developed a
variety of tools to measure an aspect of health literacyindividuals’ reading
comprehension in a medical context. Assessment of Health LiteracySpanish and
English (SAHLS&E) is a new instrument, consisting of
comparable tests in English and Spanish, with good reliability and validity in
both languages. Persons being examined in English or Spanish are presented with
18 test terms. For each term a key word with a related meaning and a distractor
word unrelated in meaning to the test term. This tests the subjects
comprehension as well as pronunciation (decoding) of health-related terms.
The Rapid Estimate of Adult Literacy in
MedicineShort Form (REALM-SF) is a 7-item word recognition test to
provide clinicians with a valid quick assessment of patient health literacy.

How can we communicate with a patient with low
health literacy? In the health field,
we typically want people to start or stop doing something or do more or less of
something. However, we often spend our resources giving people background
information instead of our recommendations. Some background information may be
important, but make sure you provide actionable information so the people you
want to reach CAN do something with the information provided.

Use of technical and
medical terminology in public communications (e.g., use of cardiovascular
disease instead of heart disease; and use of the term sodium instead of salt)

Inclusion of
statistics or terms that audiences find confusing to explain risk (e.g., high
prevalence of stroke among older adults instead of a large number of older
adults have had a stroke).

Relying on an
inappropriate mode of communication (e.g., print materials for persons with
limited reading skills).

Focusing on awareness
and information rather than action and behavior (e.g., explaining consequences
of uncontrolled glucose vs. steps to take to control uncontrolled glucose)

The patient portal is an online application that can be access
through computer or smartphone. Patient portals allows patients to have access
to their medical records, such as their lab results, previous medical visits,
medical diagnosis, and many more. For many patient portals there will be given
an access code to the patient to get into their chart, so they have view it on
any computer, or phone device. The patient portal has many advantages to it
being that the patient will not have to keep going back and forth to the
doctors office they can simply log into the patient portal, and find their
results, prescriptions, and medical diagnosis. Patient portals provide better
communication between the provider, and the patient. It is available to
patients 24/7. It makes it convenient for patients to have their medical
records at just the touch of their fingers. It is convenient to have the application
on a smart phone because if you happen to lose discharge paperwork, or if you
do not remember what you were prescribed you can open the application, and view
prescriptions that the provider prescribed. There can be some disadvantages to
patient portals because if a patient loses their phone then that could give
another person access to their medical records. One of the main disadvantages
to patient portal is security issues if there is a breach in security it could
be a HIPAA violation. They could have technical difficulties when using the
patient portal it may have some bugs, or it could have some improvements that
can be made. There could be hackers that could hack the application causing the
patients medical information to be released without their permission. For
people who are not familiar with technology it could be difficult for them to
understand how to use the patient portal. The limitations are that you cannot
leave your phone unlocked, you will have to keep logging out to ensure that your
records are secure. If patients have certain questions about their diagnosis,
or their lab results they will have to make an appointment with their provider,
so they can discuss it further. Overall patient portals are very handy when it
comes to improving patient care it can have many benefits that patients can use
to improve their health. It can give patients the opportunity to feel more in
control of their care, and have better communication between patients, and
providers.
